Forms and Document Submission Guidelines

Forms and/or documentation requested by the USCB Financial Aid/VA Office may be submitted to us via email, U. S. mail, fax or in-person.  Please include Student's Name and VIP ID on all form and documents.  For your protection, please remove all personally identifiable information (PII) such as: SSN, date of birth and driver's license number from your documents prior to submitting it.
